Issues with two identical apps in BREW app list

there's a very weird thing happened on my Sharp phone...
I loaded two different applications (App#1, App#2) on my Sharp phone via AppLoader, and I can see two different icons for these two applications; but, when I go to the BREW shop which shows the list of available BREW applications on the phone, it shows two identical (including the app name, memory usage,...) ones for App#1. and if I click on one of the icon for App#1 and remove App#1, both icons for App#1 are gone (in BREW app list) and I still can see App#2 via AppLoader... is there any issue with the EFS of my phone?
p.s. I've double checked the class ID, app name, is there anything else I might have missed that cause this problem?

How did you have the apps structured on the phone?

- app2.mif
+ [app1]
- app1.bar
- app1.mod
- app1.sig
+ [app2]
- app2.bar
- app2.mod
- app2.sig

The phones tend to be fussy about case, and require the apps to be in seperate directories (that match the name of the app).
