repeat icons under Main Menu/Brew Apps

Does anyone know how to solve the problem of repeat icons under the Main Menu/Brew Apps section? When I upload a game, three icons appear in this section that supposedly refer to the game. Only one icon actually loads the game. The other two icons display "Unknown Error (1)."

Was this app downloaded over the air, or was it loaded via serial cable?

Do the other icons that appear, come from the mif file of other modules?

Which device are you using and build # ( if emulator, which device file? )

I have not seen such behavior previously.
